I want to become a patient at Bee Well. Where do I start?

To start the Intake process, please complete our Intake Form, located here.

I have completed the Intake Form, what's next?

Once you have submitted your Intake Form, it will be forwarded to our team of providers for review. We will then contact you via text or email to specify if we are able to meet your level of care within 24-48 hours.

If accepted as a patient, you will receive our Welcome packet via email. You then have the option to schedule an Intake meeting with our on-staff nurse, or you may complete the checklist at home. Once complete, we will be able to schedule you for your first appointment!

How do I complete the Welcome Packet?

We must receive all items in the Welcome Packet's checklist prior to scheduling your first appointment with us. In order to ease this process, we offer two methods to completing your Welcome checklist:

You may choose to complete all the required steps yourself.
You may choose to schedule an Intake meeting to complete all the required documentation with our staff nurse.
